Create icon in toolbar

Create icon in toolbar was created by thijscl

In the template "Jp-custom" i'm trying to create the same Toolbar setup as in the demo.
So at the left side the icon with telephonenumer and an icon with emailaddress.

But i don't now how it's configured. Are the phone number and the emailaddress 2 seperate modules "html"?
And how can i get that icon before the text.
